Childs Plan

A personalised child’s plan is produced when a child requires a range of extra support planned, delivered and co-ordinated.

The Childs plan will detail a list of what should improve for the child, the actions to be taken and why the plan has been created. A childs plan is a legal document.

The child's plan is managed by a lead professional, Usually the childs health visitor if they are under the age of starting school. The lead professional is someone with the skills and experience to ensure that the plan is managed properly.

The child and their parent(s) will be informed about what information is being shared, with whom and for what purpose, The parents views will be taken into account in all decission making in the best intrests of the child. (unless there is a concern for the safety of a child or someone else)
